STEP 1
Determine Eligibility
If you are interested in learning more about Daymark Living as an option for your loved one, you can either call us at (972) 525-2727 or complete our contact form to get started.

Before being considered as potential Daymark Living residents, applicants must:
Be a minimum of 18 years old
Have a primary diagnosis of intellectual or developmental disability or cognitive delay
Be free of aggressive or threatening behavior
Have the ability to ambulate and transfer independently
Have an interest in participating and being an active community member
STEP 2
Tour Our Campus
After connecting with our staff, we’d like to meet you in person! Bring your family to come and tour our beautiful community, meet our staff, and truly get a feel of what life at Daymark is like.
STEP 3
Apply
If your family decides Daymark is the right next step for your loved one and all of our residency requirements are met, we will email over the link to complete our online application.

Application Fee: $500 (nonrefundable)
Community Fee: $1,500 (nonrefundable)
Security Deposit: $500
Monthly Fee: $4,900
STEP 4
Submit Records
Our admissions team may ask for educational, medical, and other relevant records to get to know your loved one before their admission interview with Daymark.
STEP 5
Admission Interview
All application documents are thoughtfully and thoroughly reviewed by our team and an admission interview is scheduled between the potential resident, their family, and our admissions team.
STEP 6
Placement Decision
Following the interview, Daymark’s admissions team will make a placement decision and notify your family.
STEP 7
Intake
If the applicant is accepted the Director of Community Life will schedule an intake meeting and move-in date. At intake, your family will meet with each department to learn how we can best serve your loved one at Daymark!
